New Patient Registration

Our practice ethos focuses on the prevention of dental problems and preservation of your teeth for life. At your first visit, we will ask you to complete a form which covers some basic details, including a medical and dental history. Please remember to bring details of any medication you are currently taking (a copy of your prescription is fine). You will then have a consultation with your dentist with digital dental X-rays which helps to determine how healthy your mouth is. At the end of your appointment, your dentist will provide you with a full report on your oral health and discuss and explain any proposed treatment which will also be written down on a treatment plan for you to take home.
